According to http://isuprod.blob.core.windows.net/media/177891/2014-22-08-wyog-2016-qs-isu-figure_skating-en.pdf all places for Men and Ladies except 4 and for Pairs and Dance except 3 are distributed on results of JrWorlds-2015, remaining - on standing at JGP-2015 (i.e. after 11 Oct 2015).

On results of JrWorlds-2015:
Men
2 spots - Japan, China, USA.
1 spot - Russia, Latvia, Israel, Korea, Argentina, Canada.
And 4 will be distributed after all seven JGP Events.

Ladies
2 spots - Russia, Japan, Kazakstan
1 spot - USA, Korea, Latvia, Slovakia, Armenia, Finland.
And 4 will be distributed after all seven JGP Events.

Pairs
2 spots - China, Canada, Russia
1 spot - USA
And 3 will be distributed after all seven JGP Events.

Dance
2 spots - Russia, USA, Ukraine
1 spot - Canada, Poland, France
And 3 will be distributed after all seven JGP Events.

Also Norway as Host Country has the right to have 1 place in all 4 disciplines.
If Norway will not qualify at JGP and will solve to send an athlete, number of qualified by JGP will be 1 less than written above.

Seems that mixed teams -- i.e., with skaters from multiple countries assigned to each of the teams -- will compete in the team event :).

Have the YOG had a team event before? With mixed teams? Or will mixed teams be new in 2016?
The same mixed teams were at 2012.

Gold got team of Shoma Uno (Japan) and unknown Lady (USA) and also unknown Ice Dance couple (Belorussia).

It says medal placements at junior worlds receive 2 entries to Youth Olympics, not the top 3 NOCs placements. I thought the extra placements would be given out via the JGP.

See http://isuprod.blob.core.windows.net/media/177891/2014-22-08-wyog-2016-qs-isu-figure_skating-en.pdf part D.
In all disciplines 6 skaters/couples are from "medal places" NOCs at JrWorlds-2015, two per NOC. I.e. for three NOCs with highest places.

The same as 14 places at JGP - for three NOCs with highest places at JrWorlds.

Does anyone know if countries that have already earned a quota place via Jworlds could earn a second quota
place via JGP 2015?

No.
For Olympics (both main and Youth) addidional quota is only for countries which have no quota from Worlds or JrWorlds.

For Men and Ladies, Members/NOCs with the 4 top ranked competitors in the final ranking of the ISU Junior Grand Prix of Figure Skating series season 2015/16 (as published after the last individual Junior Grand Prix event of Zagreb), if not already having earned a YOG Quota Place in the respective disciplineas per summary in point 1 above, will earn one (1) YOG Quota Place for the concerned discipline.

Does anyone know who among the American girls are old enough for this event? I want to see the US actually send someone good, as opposed to 2012....

From Russian pairs there is one more pair - Vygalova/Parkman are also the right age. Their only problem is that he started pairs this summer. Can male partner learn pairs within one season and still be competitive? He is probably good at jumps because he has been skating singles, but that won't help them if Vygalova doesn't have triples.
